Human Kidney Functions
Waste excretion: Kidneys remove the waste products of metabolism (urea, uric acid, creatinine).
Acid–base balance: Kidneys regulate pH by eliminating excess hydrogen ion concentration and controlling composition of the blood. Blood plasma pH is maintained by the kidney at a neutral pH of 7.4. Urine is either acidic (pH 5) or alkaline (pH 8). Potassium and phosphate require renal control as well.
Blood pressure control: Sodium ions are controlled in a homeostatic process involving aldosterone, which increases sodium ion absorption in the distal convoluted tubules. When blood pressure is too low, renin is secreted by cells of the distal convoluted tubule. Renin acts on the blood protein, angiotensinogen, converting it to angiotensin I. Angiotensin I is then converted by the angiotensin-converting enzyme in the lung capillaries to angiotensin II, which stimulates the secretion of aldosterone by the adrenal cortex. Aldosterone stimulates increased reabsorption of sodium ions from the kidney tubules, which causes an increase in the volume of water that is reabsorbed from the tubule. This increase in water reabsorption increases the volume of blood, which ultimately raises the blood pressure.
Plasma volume and osmolality
Hormone secretion: Erythropoietin is secreted for red blood cell production; deficiency is common in chronic renal anemia.
Vitamin D3 is converted from an inactive form (D2) for calcium:phosphorus homeostasis; the final stage of conversion of vitamin D to its active form, 1,25-dihydroxyvitamin D occurs in the proximal tubule.يعني تحول فيتامين د الى الصورة النشطة
Carnitine synthesis: Carnitine carries fatty acids from cytoplasm to mitochondria for heart and skeletal muscle fuel. Lysine, methionine, vitamin C, iron, vitamin B6, and niacin are needed to produce carnitine.الكارنتين دا اصلا له وظيفة في انتاج الطاقة
Glucose homeostasis: The kidney plays a role in gluconeogenesis and glucose counterregulation.توازن السكر في الدم

طبعا النورمال GFR للشخص الطبيعي هو انو يعمل فلترة ١٠٠ مل كل دقيقة
طبعا مرضى الكلى تحاليلهم تختلف عن الناس الطبيعين يعني مثلا النورمال يكون تحليل الفوسفور عندهم من 2.5 - 4.8 mg\dl اما بالنسبة لمرضى الكلى تحليل الفوسفور عندهم يختلف عن الاشخاص النورمال حيكون من 3-6
عشان كدا لازم نحفظ اللاب ريزلت وهي مع الممارسة راح تنحفظ بس الشاطرة تحفظها من غير شي
طبعا مرضى acute renal failure دول نديهم بروتين قليل لانو دول مو دايما يغسلوا فنديهم بروتين قليل عشان نقلل overload على الكلى
فاكتور البروتين لهم 0.8 - 1 طبعا نضرب في IBW
بالنسبة لمرضىى chronic renal failure دول نديهم بروتين اعلى لانو هما تقريبا كل ٣ ايام بيغسلوا فمافي مشكلة لو ادينا برووتين عالي
يعني الفاكتور يكون 1.2 وطبعا نضرب في IBW
في معلومة خاطئة ايام الجامعة اخدناها وحدة من الدكاترة مدري من فين جابتها؟؟!!!
المعلومة هي : انو مريض الكلى لمن يغسل نديلو بروتين عالي لكن باقي الايام نديلو بروتين قليل؟؟؟!!
صراحة مافي اي مرجع لدي المعلومة دورت في الكتب والمواقع مافي شي بيقول دا الكلام
اللي تلقى دي المعلومة ياريت توريني هي
بنات انسو اللي اخدناه في الجامعة وحتى اللي اخدناه ارجعوا اتأكدوا منو كتييييير معلومات بتكون غلط او قديمة ؟ حتى لو كانت الدكتورة قد الدنيا ممكن تغلط في بعض المعلومات ولا تنشروا معلومة الا وانتو متأكدين منها
يعني مثلا شوفتي دكتور في الانستغرام او في اي بطيخ قال شي بخصوص التغذية روحي اتأكدي لا تاخدي المعلومة وتقولي دكتور مشهووووووووووور اكيد كلامو صح ؟؟؟؟!!!
ترا عادي يكون دكتور مشهور وحمار في نفس الوقت مافي مشكلة لانو مو كل الناس عندهم امانة وممكن يكون الدكتور يغلط بعض الاوقات وممكن تكوني انتي فهمت المعلومة غلط من الدكتور
اتمنى اني في يوم اشوف الناس تدور على المعلومة قبل ما تقول "دكتور قال ............."
